INTRODUCTION
This Consent Form is intended to outline the terms and conditions of parent coaching services provided by the Coach. Parent coaching is a supportive, non-judgmental process that helps parents develop strategies to enhance their parenting skills, improve family dynamics, decrease stress, improve communication, and generally support the well-being of their children. The goal is to empower parents with the tools and knowledge to foster a positive home environment.
SCOPE OF SERVICES
Parent coaching is not a replacement for therapy, counseling, or any form of medical treatment. Parent coaching is not provided to you by a licensed medical provider, counselor, or therapist. You acknowledge you are not entering into a patient-therapist relationship by agreeing to participate in coaching. Instead, coaching is a collaborative partnership aimed at enhancing parenting strategies and skills. Specific topics covered may include:
• Communication with children
• Positive discipline techniques
• Emotional support strategies
• Building a stronger parent-child relationship
• Planning and organization
You acknowledge that coaching does not involve the diagnosis or treatment of mental disorders as defined by the American Psychiatric Association and that coaching is not to be used as a substitute for counseling, psychotherapy, psychoanalysis, mental health care, treatment, or other professional advice by legal, medical or other qualified professionals and that it is the your exclusive responsibility to seek such independent professional guidance as needed. If you are currently under the care of a mental health professional, it is recommended that you promptly inform the mental health care provider of the nature and extent of the coaching relationship agreed upon by you and your Coach.
Coaching services may be provided via online video platforms (e.g., Zoom, Google Meets) or via text messaging. You understand that guarantees of privacy are not 100% with these platforms but your coach and employees of Joon will make every effort to ensure the confidentiality of your communications with your coach. You also understand that our coaching services are not a substitute for acute or crisis services. As a result, our coaches may not be available at all hours, on weekends, evenings, or holidays. They may not respond right away to your messages and there will be times when they are not available. You agree that if you are in an emergency, you will contact 911 or visit the nearest emergency room for support.
You also agree that you can be contacted via email, text messaging, phone call, or other means of communication to receive optimal services from your coach. This may include leaving messages on your voicemail. We encourage you to enact privacy controls on your phone to optimize privacy.
CONFIDENTIALITY
All information shared during the coaching sessions will be treated as confidential and is not intended to be shared with anyone outside of Joon Health. Sessions may be audio or video recorded for quality assurance and to inform future products and/or service development. The Coach agrees to respect and protect the privacy of the parent(s) and children involved, with the following exceptions due to ethical and legal reasons:
1. If the Coach is legally required to report any suspicion of abuse, neglect, or harm to a child, elderly person, or a vulnerable adult confidentiality may be broken to protect you, your child, or someone else.
2. If the Coach is subpoenaed by a court or other legal body to provide information we are legally required to comply with that subpoena.
3. If there is an immediate risk of harm to any individual, including self-harm or harm to others confidentiality may be broken and steps may be taken that involve loss of confidentiality.
This coaching relationship, as well as all information (documented or verbal) that the Client shares with the Coach as part of this relationship, is bound by the principles of confidentiality. However, please be aware that the Coach-Client relationship is not considered a legally confidential relationship (like the medical and legal professions) and thus communications are not subject to the protection of any legally recognized privilege. The Coach agrees not to disclose any information pertaining to the Client without the Client’s written consent. The Coach will not disclose the Client’s name as a reference without the Client’s consent.
RESPONSIBILITIES OF THE PARENT
As the parent/guardian, you agree to:
• Actively participate in coaching sessions and apply suggested strategies to the best of your ability.
• Maintain open and honest communication with the Coach.
• Attend scheduled sessions promptly or provide 24-hour notice if rescheduling is necessary.
RESPONSIBILITIES OF THE COACH
The Coach agrees to:
• Provide support, guidance, and recommendations in a professional, non-judgmental manner.
• Tailor coaching strategies to the specific needs of the parent(s) and child(ren).
• Maintain confidentiality as outlined above.
CANCELLATION AND RESCHEDULING POLICY
To ensure that coaching sessions are productive, please provide at least 24 hours’ notice if you are unable to attend a scheduled session. If you do not provide at least 24 hours prior to cancelling a session or fail to arrive for an already scheduled session, you may lose that session for the month as part of your monthly subscription. You may also be charged a $30.00 no-show/late cancellation fee. If this occurs 3 or more times, your coach may discuss with you the appropriateness of continuing parent coaching services at this time.
RISKS AND BENEFITS
While parent coaching has many benefits, it is important to understand that it may also bring up challenging emotions and require difficult changes. Positive outcomes are more likely when the parent is committed to the process and willing to work on the areas discussed. It may also not work in every situation and success is not guaranteed. It is important to keep in mind you may not see improvements from this program. You acknowledge that your coach will not be on-call and may not be available or capable of helping in an emergency situation. If such a situation arises, you agree to contact 911 or visit the nearest hospital for more intensive services. Your coach may not respond to your messages right away or outside of work hours.
CONSENT AND AGREEMENT
By signing, you acknowledge that you have read, understood, and agree to the terms outlined in this Consent Form. You also confirm that you understand the nature of parent coaching and that it is a collaborative process designed to enhance parenting skills.
